I actually went to Antarctica with my school so it was all pre-planned. The most difficult part is deciding on the company to go with, everything else is taken care of. You sleep on the boat, and the landings are choosen by the crew for it's wildlife, historical value, or exceptional beauty. Some cruises do stand-bys where you can get a super cheap deal a few days before the ship leaves. We had an Aussie on our ship who did it that way. Most cruises leave from Ushuaia, Argentina.
